ConnectMyCare allows you to take control of your therapy and disease state by providing you with tools to manage who, what, when and how in a central location – on your phone or tablet. The application is for patients on specific drug programs and can only be accessed by patients that have been given a specific username and password from participating pharmaceutical manufacturers.

FEATURES

• My Nurse allows you to speak to your program nurse at a single touch. You can choose to e-mail, call or chat with your assigned nurse—who has specific knowledge of your disease and therapy. Nurses have access to your symptoms, journal, medications list, resources and provider list and can provide you with real-time help and feedback based on your therapy and the information that you are providing via the application.


• My Symptoms contains a list of common symptoms associated with your therapy and disease. You can create symptoms logs to track your symptoms over time. Some symptoms have follow up questions to obtain specifics on what you are experiencing.

• My Journal allows you to track information about your health and lifestyle each day. The information that you track is focused on your specific disease state and allows you to record your daily wellness.

• My Appointments gives you the opportunity to input all of the appointments and events associated with your care in one location. View the calendar to see all upcoming appointments. Reminders will show at the top of your home page.

• My Medications is the area where you can create a list of all medications you are currently taking, as well as create reminders that will show on your application home page.

• My Providers page gives you the ability to create a comprehensive contact list of your doctors, hospitals, medical personnel and sites of care for you to reference in one location. Add contact information and categorize your contacts. Your nurses will be able to add nearby specialists for you as well.

• My Resources contains a repository of helpful websites, emails, phone numbers and images that support you in managing your therapy. Your nurse can add resources to this page, or you can add your own.

• My Questions contains categorized FAQs to provide quick answers to the uncertainties of managing your disease.
